Transcript Request

  • Students should use this form to request official transcripts from the University of Cincinnati. The forms should be submitted to the Office of Student Records. Click here for more information.

Registration Change Form

  • This is the form that must be submitted to the registration office in order to add or drop classes after the appropriate deadlines have passed. See page 1 of the current learning opportunities for a list of these deadlines. Click here to view this form.

Academic Fresh-Start Application Form

  • This form is used when students want to apply for an academic fresh start. There are many stipulations to the Academic Fresh Start Policy, so make sure to see an advisor to find out if you meet the criteria. Click here to view this form.

Consortium Cross-Registration Authorization

  • This form is used for taking classes at other greater Cincinnati colleges that are not offered at UC. The class must be approved by an academic advisor in the student's home college. Click here for more information.

Complete Withdrawal

  • This form is used for students who want to completely withdraw from the university. Students must meet with an advisor to fill out this form. Click here for more information.

Application for Change in College and Program Form

  • Students should use this form when they want to change colleges within the University of Cincinnati. The form should be submitted to the college office for the college they want to transfer to. Click here to view this form
