Clinical Education Requirements

Immunizations and Physical Exam

Students in the Doctor of Physical Therapy program are required to have the following vaccinations and an annual physical exam.

  • Hepatitis B (postive titer)
  • Influenza (flu)
  • Measles, Mumps, Rubella (MMR)
  • Meningococcal
  • Tuberculosis
  • Tetanus, Diptheria, Accellular Pertussis (Tdap)
  • Varicella (or proof of immunity via IgG titer)
  • COVID-19 vaccination

Notes:  

  • Hepatitis B:  You will need a positive titer AND proof of a vaccination.  Start early in case you need the series.
  • Tetanus is needed for cadaver lab (Summer 2025: Gross Anatomy course).
  • For the MMR and Varicella you will need either a positive titer or proof of vaccination.

The university has adopted a new platform for immunizations - Bearcat Health App.

Please download the form below, visit your doctor for the required physical exam and titers (form must be signed by MD or CNP).   As this is a new process, thoroughly read the Health Sciences Student Immunization Requirements.

**Please note:  When uploading the immunization form to the Bearcat Health App, the physical exam form should NOT be uploaded.  No additional documents can be uploaded with the Student Immunization Form (including a vaccination printout from your healthcare provider). 

Please hold on to the hard copy of your Physical Exam Form.  Our Clinical Director will provide instructions on our Exxat system which is used for our clinical education component of the program.
